Sports Physicals for School Year 2009/2010

Attention McKean Parents and Athletes…
Interested in participating in fall sports for the 2009-2010 school year? If your answer is yes, then you will need a sports physical prior to practice starting on August 15, 2009. The McKean Wellness Center will be closed over the summer and will reopen to schedule sports physicals after August 24, 2009, therefore; in the meantime, here is a list of alternative locations.
Alternative Locations for Sports Physicals
Christiana Care Family Medicine Center – Wilmington Annex will offer three evenings of sports physicals. The office will be open from 4 – 8 pm for sports physicals: July 20 and 27 and August 3. The Annex is located at 1400 N. Washington Street, Ste. 300; Wilmington, DE 19801. You can schedule a physical by calling Valerie at 302-477-3325 after July 1.
Rockwood Family Medicine provides comprehensive care to patients of all ages. We are located at 2002 Foulk Road, Suite C in North Wilmington, near Brandywine High School. We will be offering sports physicals throughout the summer and in most cases can schedule students on the same day they call. Please contact our office for an appointment at 302-529-1975. Please make sure to tell the staff that this is for a sports physical.
General information
The cost for a sports physical is $45. This may be paid by cash, credit or check at the time of service.
The required DIAA form must be completed and signed prior to the appointment time.
Parents or guardians do not need to be present for the exam since the DIAA form includes consent for the pre-participation exam.

Click Sports Physicals to go to description and list of providers. Click on Pre-participation physical evaluation to get to the medical forms required for the exam. Then click “Medical Forms” and then click “All Forms” if you did not receive a copy of the DIAA form from your school or wellness center.

Take Care Clinic’s at the Chestnut Run Shopping Center 1710 Faulkland Road, Wilmington, and at 216 Suburban Dr. in Newark. Take Care has a special promotion taking place this summer "Fiscally Fit during the summer", from now until September 30th sports and camp physicals are only $30.00. There is no appointment needed. What is requested is that the parent or guardian accompany the child and that prior to signing in the sports or camp forms are completed by the student and parent.
